sankalpam to combine masikam for previous missed masikam

Can anyone please guide on what the procedure should be to do masikam combining previous month's and the current month? What are the changes in sankalpam and other procedures?

How can you do it without a vadhyar?
All procedures are same except sankalpam.
"...... adheetha .... anumasika shradham adhya ..... anumasika shradham cha adhya karishye" fill the masikam number in dash.
One pitru swami extra to be placed for every additional masikam combined.
